If you notice any errors in the directory, please fax, on practice letterhead, the information as it should appear, to the attention of Provider Services at 1-833-350-3477, Monday through Friday, 8 a.m. – 8 p.m., from April 1 to September 30; or seven days a week, 8 a.m. – 8 p.m., from October 1 to March 31.
